Zakat Request Form & Policy

Al-Farooq Masjid has been blessed with the ability to help families in need when Zakat funds become available. To request Zakat, please complete the below form in its entirety. Incomplete forms will not be considered. If you have any questions or to follow up on your request, please send a text message to 404-874-7521.

Please note the following important guidelines:

• This form is only for consideration of Zakat and does not guarantee that assistance will be provided.
• A member of management will contact you via text message at the phone number provided in your application when funds become available or if additional information is required. Please allow 10–15 business days to receive help or to be contacted by us.
• Zakat will only be distributed in check form. No cash will be distributed — no exceptions.
• A government-issued photo ID is required when receiving assistance.
• We do not provide any transportation, nor do we cover transportation expenses. Do not ask for cash for bus fare or other transportation costs when collecting assistance. It is your responsibility to arrange coming to and leaving the Masjid.
• Zakat will only be distributed to those currently residing in the Metro Atlanta area.
• Zakat will not be distributed to any organization.
• A completed form is required to receive any assistance.

Due to the high volume of applicants, we are only able to provide one-time help per individual or family. Repeat applicants may not receive assistance again.
